Casusbespreking therapieresistente depressie

1. Kenmerken en classificatie
Diagnose depressie: DSMIV (definitie van psychiatrische aandoeningen). Minstens 5 symptomen
waarbij 1 en/of 2 noodzakelijk gedurende minstens 2 weken.

1. Depressieve stemming.
2. Apathie (Verlies van interesse en plezier)

3. Gewicht/eetlust verandering.
4. Slaapstoornis (insomnia = niet kunnen slapen of hypersomnia=overmatig veel slapen)
5. Psychomotorische agitatie of retardatie
6. Vermoeidheid.
7. Gevoel van waardeloosheid, schuldgevoelens
8. Concentratiestoornis, besluiteloosheid
9. Suïcidale gedachten.




Kwantitatief beoordelen van de ernst van depressie met de hamiltonschaal (HRSD= Hamilton rating
scale for depression) HDRSD of Ham-D:

0 – 7: geen depressie
8 – 13: marginale depressie (subthreshold)
14 – 18: lichte depressie (mild)
19 – 22: matige depressie (moderate)
> 22: ernstige depressie

Non-respons: ≤25% reductie van de sterkte van de symptomen t.o.v. baseline.

Partiële respons: 26-49% reductie van de sterkte van de symptomen t.o.v. baseline.

Respons: ≥50% reductie in de sterkte van de symptomen t.o.v. baseline.

Respons met residu symptomen: respons met partiële remissie.

Remissie: geen symptomen of erg weinig. Gedefinieerd door een absolute score (vb. HAM-D totale
score van ≤7) en verbetering van psychosociale en functioneren op het werk.

,Therapieresistente depressie (TRD): depressieve episode waarbij niet gereageerd wordt op
behandeling. Het niet verbeteren van depressie ondanks proberen te behandelen met 2 adequate
antidepressiva in de geadviseerde dosis en met geadviseerde duur.

Risicofactoren voor TRD:

 Comorbide angststoornissen (vooral paniekstoornis en sociale fobie)
 Comorbide persoonlijkheidsstoornis
 Suïcidaal risico
 Hoge mate van ernst van de symptomen
 Aanwezigheid van depressieve toestand
 >1 eerdere hospitalisatie
 Terugkerende episoden
 Non-respons op het eerste antidepressivum
 Leeftijd van optreden ≤ 18 jaar
 Lagere GABA concentraties in de hersenen
 Een aantal polymorfismen in het gen dat codeert voor het serotonerge systeem.
 Alcohol en middelen gebruik (ook op laag niveau).
 Psychoses (psychotische depressie)
 Pijn
 Sociale stressoren (stress hypothese HPA-as)
