REASON FOR ENCOUNTER BUMPS ON MY
FACE ACTUAL 100% 2025
Chief Complaint (CC):
"Bumps on my face" — reported by the patient, present for the past few weeks.
History of Present Illness (HPI):
The patient is a 14-year-old who has come in with concerns about bumps on their face, mainly affecting
the forehead, cheeks, and chin. These lesions started about 3 to 4 weeks ago and have been increasing
in number. The bumps are described as red, sometimes featuring whiteheads, and can be tender or
itchy at times. While the patient doesn’t report any significant pain, they do feel self-conscious about
their appearance and are embarrassed at school.
The patient hasn’t used any prescription treatments but has tried an over-the-counter acne face wash
with little success. They’re not sure what might have triggered the breakouts, although they’ve noticed
their face feels oilier lately, and the breakouts seem to get worse around the time of sports practice.
There are no signs of fever, fatigue, or other systemic symptoms. The patient hasn’t made any recent
changes to their diet or medications and has no known allergies.
