, Holistic, Person-Centered Plan of Care for Gordon Brune
Profile of Gordon Brune
This patient is a 55-year-old male, here at the Neighborhood Clinic, with a diagnosed case of
diabetes. He says he has a problem with high blood glucose levels because of his bad diet and lack
of exercise. His occupational status as a desk job holder means he has little chance of moving
around. Mr This makes her feel lonely because she lives alone and has few people to support her.
He writes about the severe emotional strain associated with the disease and its interference with his
life. Also, he complained that he did not know enough about caring for himself properly.
Dimensions of Wellness Influencing Gordon Brune
1. Emotional:
Mr Brune’s mental health is not in good condition. He said he gets stressed by his health
complications, and this affects his chances of following through on recommended treatment.
Emotional instability is an important predictor of poor glycemic control and decreases his
willingness to change his behaviour.
2. Physical:
Gordon has uncontrolled diabetes type II, which has symptoms of fluctuating blood sugar levels.
Lack of exercise increases his chances of developing long-term complications of the disease, such
as neuropathy and cardiovascular disease due to ‘junk’ food.
