Explain the purpose of a coordination of care from and when it might be used.
What are the risks of letting client data leave the office?
Hello class and professor,
First, care coordination is organizing patients care by giving information to other
providers of the patient with the goal of giving the patient better care
(SAMHSA, 2022). Looking at google for examples of these forms, most of them
seem to include patient information, some degree of clinical information, and
who the health care providers are. I think a time that it could commonly be
used for is if a professional is unable to prescribe medications, they would use
this form to have a primary care physician or doctor prescribe medications
based on the professional's diagnosis. I think there are a lot of confidentiality
issues with letting client information leave the office. Emails are easily hacked
into, mail gets stolen all the time, text messages and phone calls could go to
the wrong person or be seen by others (although I do not believe this should
even be considered a method for transferring confidential information), and if
people still use them, fax machines could accidentally have the wrong number
typed on them.

to Mental Health, psychotherapy notes receive extra protections because they
have extra sensitive information and they are only used by the professional
that made them (Health and Human Services, 2020). This pdf also says that
