counter..until..discharge...Each..depart..will..check..off..that..the..form..is..co
mpleted..by..the..previous..team..and..that..no..information..is..missing...Thi
s..form..will..have..all..information..needed..for..each..department..to..get..in..
touch..with..the..guardian...The..form..will..start..with..Pre-
op..department..with..information..from..registry.
• There..needs..to..be..special..considerations..for..minors..during..the..pati
ent..encounter..and..guardianship..needs..to..be..established..in..the..regi
stry..department.
• If. . Registry. . does. . not. . have. . all. . patient. . information. . needed,. . they. . must. . re
ach. . out..to. . the. . Doctor’s. . office,. . or. . retrieve. . it. . from. . the. . patient. . before,. .
the. . patient. . is..allowed..in..pre-op.
• Security..will..work..with..leadership..to..create..arm..band..identification..prot
ocols.. . The..band..will..be..worn..by..the..patient..and..the..guardian...If..they..d
o..not..have..an..armband..on,..they..are..not..allowed..beyond..the..waiting..area

• Security..must..identify..visually..(in..person)..that..both..armbands..are. . matc
hing,..and..are..both..worn..by..the..patient..and..the..guardian,..with..the..recove
ry..nurse..present.. . They..must..have..the..guardian..then..sign..the..patient..out.
