Obtaining the History
A thorough and systematic history forms the cornerstone of Family Nurse
Practitioner (FNP) practice. By carefully collecting patient information, you
can generate an accurate differential diagnosis, guide your physical exam,
and build meaningful rapport with both patients and their families.
Chief Complaint (CC)
The chief complaint is the primary reason for the patient’s visit,
stated in their own words.
It should be concise and specific, such as “sore throat x 2 days”
or “annual physical.”
In pediatric or geriatric cases, the chief complaint may be
provided by a caregiver or family member.
History of Present Illness (HPI)
The HPI is a chronological and detailed description of symptoms relating to
the chief complaint. Structure your questions using the
OLDCARTS/OPQRST framework:
Onset: When did the symptom begin?
Location: Where is the symptom located?
Duration: How long has it lasted? Is it continuous or episodic?
Character: What is the symptom like (sharp, dull, burning)?
Aggravating/Alleviating factors: What makes it worse or better?
Radiation: Does the pain or symptom travel to other areas?
Timing: Is it constant or intermittent? Are there patterns related
to time of day, meals, or activity?
Severity: Quantify the symptom (e.g., pain scale 0–10) and
describe its impact on function or activities of daily living
(ADLs).

Pediatrics: Parent observations are essential, including details
about behavior, feeding, sleep, school performance, and recent
exposures.
Geriatrics: Screen for cognitive impairment, allow extra time
for the interview, verify details with caregivers as needed, and
assess for falls, polypharmacy, and sensory deficits.
Past Medical History (PMH)
Document chronic conditions such as asthma, diabetes, or hypertension, as
well as significant childhood illnesses.
Include relevant details on hospitalizations, surgeries, injuries, and
transfusions.
Gyn/Ob(GTPAL (G, T, P, A, L).): When applicable, record information on
menarche/menopause, menstrual patterns, contraception, STI history,
gravida/para/abortus, and pregnancy or lactation history.
Document psychiatric history, including diagnoses, hospitalizations, and
therapy.
Medications
List all prescription medications, over-the-counter drugs, vitamins, and
herbal supplements.
Provide dose, route, frequency, duration, and any recent changes.
Assess medication adherence, financial or coverage barriers, and
perceptions of benefit.
Screen for polypharmacy and potential drug interactions, particularly
among older adults.
Record any adverse effects and prior treatment failures.
Allergies
Evaluate for allergies to medications, foods, environmental factors, and
latex.
