4 Domains of good medical practice

-Explain why Junior Doctors may be experiencing stress during their work (many
different types of stress encountered, for example working antisocial hours, dealing
with psychologically and mentally challenging situations, balancing a career with your
personal life, studying for postgraduate exams and having a fear of making a mistake)
which may lead to burnout, a syndrome that encompasses three dimensions:
emotional exhaustion, depersonalisation and reduced personal accomplishment-I
have acknowledged these stresses for junior doctors through additional reading,
including a postgraduate medical journal published by the BMJ investigating the
impact of debriefing on junior doctor stress, which was inconclusive as they had a
small sample size, but showed junior doctors appreciated the value of debriefing as a
source of emotional and social support.-Before any steps are taken, junior doctors
have to themselves first acknowledge their stress and that they would like to improve
it. The can then start creating job lists, delegating tasks to different team members and
asking for help when they recognise a problem out of their expertise which are all

, ways junior doctors can deal with immediate stress - but is not a long term solution-
One of the main long term solutions is how doctors relax outside of the work
environment, for example through sports, socialising and talking with friends-It is very
important to talk about stressful situations, such as an emotionally difficult case at
work. This prevents people from bottling up their emotions, and by talking with other
Doctors they are speaking with those who truly understand their situation. In a recent
seminar I attended from Brighton and Sussex medical school, a GP trainee used a
metaphor which I really resonate with. She described having a 'sad bad basket' in
which you take on little parts of your patients problems when you empathise with
them and carry them with you, and the importance of emptying the basket at the end
of the day in order to help new patients with an empty basket. If the basket gets too
full, it may weigh you down and then cause burnout. This is why it is so important to
practice self care and seeking support, to avoid burnout and ultimately protect
patient safety.

How do you think Junior Doctors cope with stress in the workplace?

-established as part of the Health and Social Care Act in 2012, and replaced Primary
Care Trusts on 1 April 2013.
-CCGs are groups of general practices (GPs) which come together in each area to
commission the best services for their patients and population.
-they buy services for their local community from any service provider that meets
NHS standards and costs eg. NHS hospitals, social enterprises, voluntary
organisations or private sector providers. This means better care for patients,
designed with knowledge of local services and commissioned in response to their
needs.
-CCGs commission a wide range of services including mental health services, urgent
and emergency care, elective hospital services, and community care.
CCGs are responsible for about 60% of the NHS budget, they commission most
secondary care services, and play a part in the commissioning of GP services
