How did the simulated experience of Mary Richards’s case make you feel? I was a little worried, because I knew Mary Richards was experiencing digoxin toxicity. She also seemed very confused about the meds she was taking, so it made me uneasy. Talk about what went well in the scenario. I feel as if I did all my assessments thourougly. I did a medication reconciliation before I knew it was required. I gave the right medications and checked the dose with another nurse beforehand. Reflecting on Mary Richards’s case, were there any actions you would do differently if you were to repeat this scenario? If so, how would your patient care change? I ended up getting a 100 on the second try, so I do not believe I would do anything differently. Scenario Analysis Questions1 PCC/EBP/PCC What priority problem(s) did you identify for Mary Richards? Her reported nausea, dizziness, weakness, abdominal pain and vision issues. Her potassium was low. I think digoxin toxicity would be the priority problem. PCC/EBP/PCC Are there any red flags noted when looking at the medications that Mary Richards is taking? Why or why not? Yes there were red flags. Mary Richards could not remember what medications she was taking or if she had taken them at all. PCC/EBP/PCC What are the potential problems that could arise if Mary Richards does not receive treatment immediately? What type of treatment would she benefit from? Digoxin toxicity can cause fatal arrhythmias. Mary Richards could die if leh untreated. She should receive potassium. She also needs to learn how to keep up with her medication regimen. She should keep track of the medicines she is taking and when she is taking them. T&C What other interprofessional team members should be involved in Mary Richards’s care? She should be seen by a cardiologist and pharmacist. She should follow up with her primary care provider. Concluding Questions Describe how you would apply the knowledge and skills that you obtained in Mary Richards’s case to an actual patient care situation. I would know better about the side effects of digoxin toxicity. I would also know the proper treatment for this. I would know what assessments had to be made in a similar scenario.
