provide proof of fitness

use disorder- related, or addiction-related continuing education activity; and a licensee
who dispenses self-administered hormonal contraceptives under a standing prescription
order has participated in at least three contact hours of continuing education activity
related to self- administered hormonal contraceptives.
When are you exempt from CE - ANSWER between time of initial licensure and 1st

Prereq to become an inter is one of the following - ANSWER Current enrollment, in
good standing, in an approved col- lege or school of pharmacy; Graduation from a
college or school of pharmacy along with: Proof the applicant is certified by the Foreign
, Phar- macy Graduate Examination Committee (FPGEC), if applicable; or Application for
licensure as a pharmacist by exam- ination or reciprocity; or By order of the Board if the
Board determines the applicant needs intern training
If an intern stops attending pharmacy school classes w/o graduating - ANSWER shall
stop being an intern and give the BOP back the license w/in 30 days
To become an intern preceptor a pharmacist shall - ANSWER Hold a current
unrestricted pharmacist license; have at least one year of experience as an actively
practicing pharmacist; and If found guilty of violating any federal or state law relating to
the practice of pharmacy, drug or device distribution, or recordkeeping or unprofessional
conduct, enter into an agreement satisfactory to the Board that places restrictions on
the pharmacist's license.
A pharmacist or a graduate intern or pharmacy intern under the supervision of a
pharmacist shall perform the following pro- fessional practices in dispensing a
prescription medication from a prescription order - ANSWER receive oral/fax/email rx,
reduce it to writing, get the name of who called in the rx, and initial, record the pts name,
address, telephone, DOB, gender, individual hx, comments, check that the rx is okay for
the pt based on allergies, other meds, dose, and is generally okay,
compound/mix/combine rx, supervise prepackaging of drugs and initial once checked,
check data entry, final accuary check, get refills, consultation about rx
The oral consultation is required whenever the following occurs - ANSWER The
prescription medication has not been previously dis- pensed to the patient in the same
strength or dosage form or with the same directions, pharmacist decides its needed, or
pt/pt caregiver requests one
Oral consultation shall include - ANSWER Reviewing the name and strength of a
prescription medication or name of a prescription-only device and the labeled indication
of use for the prescription medication or prescription-only device; Reviewing the
prescription's directions for use; Reviewing the route of administration; and Providing
oral information regarding special instructions and written information regarding side
effects, procedure for missed doses, or storage requirements.
A pharmacist shall ensure that the follow- ing information is recorded on the back of a
prescription order when it is refilled: - ANSWER Date refilled, Quantity dispensed, Name
or approved abbreviation of the manufacturer or distributor if the prescription order is
written generically or a substitution is made, and The name or initials of the dispensing
pharmacist.
Except for a prescription order for a controlled substance, a pharmacist, using
professional judgment, may make the following adaptations to a prescription order if the
pharmacist documents the adaptation in the patient's record: - ANSWER Change the
prescribed quantity if the prescribed quantity is not a package size commercially
available from the manufacturer; Change the prescribed dosage form or directions for
