and controlled.
2. Assure the PTS is known and understood by the person performing the task.
3. Assure all people who are performing the task are aware of the risks and conform to

4. Guarantee that at the time the task begins, all the risks are identified and controlled
by the owner.
Scope - ANSWER +: PTS applies to all situations where an owner (MEL or contractor)
authorize access to another person or group of people (MEL or contractors).
-: Does not exempt the workers from assuring the safe condition of the equipment,
activity, and work area.
>>The executor must immediately notify the owner of any anomaly or failure they see or
detect.
Cases - ANSWER 1. confined space
2. pressurized rubber-lined spark-risk pipes
3. hot work, fire risk, explosions
4. process involves hazardous substances
5. x-rays or radioactivity
6. people in contact with electrically energized equipment
7. storage y/o use of explosives outside the norm
8. excavations greater than 45cm
9. excavations in foundation or reinforced concrete or slabs
10. if the superintendent deems it necessary in order to prevent physical harm of the
executors
Personnel Authorized to Sign PTS - Owners - ANSWER Owners:
MEL- L6, L7, or L8 (or those who replace them), trained by a competent body, certified
by MEL safety manager, and authorized by respective area manager
Contractors - supervisors trained on the prevention of risks, qualified by the MEL safety
manager.
EXCEPTIONS: hot work, confined spaces, excavations!
Personnel Authorized to Sign PTS - Executors - ANSWER 1. supervisors and foremen
trained on the prevention of risks and qualified by MEL safety manager
2. MEL employees trained by MEL safety manager and by request of respective
superintendent
Responsibilities - ANSWER 1. maintain and display the list of tasks and the equipment
associated
