Health screening questionnaire
Resting heart rate:
Client Clients Result Normative Data result

she 20 x 4 = 80 Below Average

him 19 x 4 = 76 Below Average

Notes: I have timed them, for 15 seconds then timed it by 4 to get it to 1 minute. From
the result she and him have got below average which means that they have worse
score than the average of the population. Also could be at risk of getting health
problems.

Blood pressure:
Client Result Normative Data result
she 106: Systolic (mm Hg) Normal
61: Diastolic (mm Hg)
him 106: Systolic (mm Hg) Normal
76: Diastolic (mm Hg)

Notes: Made sure that arm band got rapped correctly properly around the persons
biceps so that it is on the vein were you can get the pulse. she and him got normal
results which means that it is average and this means that they are in a healthy state
which there health is not at risk.

Max heart rate:
Client Client result Result
she 220 - 16 = 204 This she's maximum heart
rate
him 220- 16 = 204 This is him’s Maximum
heart rate

Notes: This was really simple to do, all it is 220 take away your age and this will show
your maximum heart rate(MHR). which means this can’t get any higher than that
when exercising because that what it can go up to. As she and him get older there
(MHR) will get lower and lower because there age changes which they get older
because when your age goes up and you must work Max heart rate it will be a bigger
number that you must take away form (MHR)
