Prototype drug: Thioridazine
Therapeutic Class: antipsychotics Pharmacological class: phenothiazines
Clinical Indications: (top three)
1. Treatment of refractory schizophrenia
2. Management of psychoses
3. Second line treatment after failure with atypical antipsychotics
Mechanism of Action: Alters the effects of dopamine in the CNS. Possesses significant anticholinergic and
alpha- adrenergic blocking activity.
Side/Adverse Effects: (adverse in bold)
• Sedation
• blurred vision
• dry eyes, mouth
• hypotension, constipation,
• photosensitivity
• Neuroleptic malignant syndrome, arrhythmias, QTC prolongation, agranulocytosis
Route and Dosage: Adults only
PO 50–100 mg 3 times daily initially; may be gradually ↑ to a maintenance dose of up to 800 mg/day.
Nursing Implications: (top 3)
1. PO Administer with food, milk, or full glass of water to minimize gastric irritation.
2. Monitor for development of neuroleptic malignant syndrome & Notify health care professional
