Oner et al
Oner et al. (2020): A Systematic Review of Nursing-
Sensitive Indicators for Nursing Care
This systematic review by Oner et al. investigated nursing-sensitive indicators (NSIs), which
are criteria used to measure the quality of nursing care. The researchers conducted a
comprehensive analysis of studies published between 1997 and 2017 to identify the most
frequently employed NSIs.

Conceptual Framework

The researchers used a conceptual framework that combined elements from three different
frameworks:

 The Donabedian Model (evaluates healthcare quality through structure,
process, and outcome measures).
 The National Quality Forum (NQF) framework (focuses on performance
measures for healthcare).
 The American Nurses Association (ANA) framework (identifies nursing-
sensitive quality indicators).

By combining these frameworks, the researchers were able to categorize NSIs into four main
groups.

Four Categories of Nursing-Sensitive Indicators

The analysis by Oner et al. revealed that NSIs can be grouped into the following four
categories:

 Organizational-focused structural indicators: These indicators assess
aspects of the nursing care environment, such as nurse staffing levels,
nurse-to-patient ratios, and the availability of resources.
 Nursing-focused process/intervention indicators: These indicators
measure the actions and processes undertaken by nurses, such as patient
education, pain management, and medication administration.
 Nurse-focused outcome indicators: These indicators evaluate the
knowledge, skills, and experience of nurses, such as their competency in
performing specific procedures or their ability to collaborate with other
healthcare professionals.
 Patient-focused outcome indicators: These indicators assess the
experiences and health outcomes of patients, such as patient satisfaction,
pressure ulcers, hospital-acquired infections, length of stay, and mortality
rates.

Most Frequently Reported NSIs

The review identified the following NSIs as the most frequently reported in the literature
between 1997 and 2017:

,  Nurse staffing levels: This indicator reflects the number of nurses
available to care for patients. The review found that higher nurse staffing
levels are associated with better patient outcomes.
 Patient mortality rates: This indicator measures the number of deaths
that occur in a healthcare setting. The review found that lower mortality
rates are associated with better quality of nursing care.
 Nosocomial infections: These are infections that patients acquire while
receiving medical care in a hospital or other healthcare facility. The review
found that lower rates of nosocomial infections are associated with better
quality of nursing care.

Overall Significance

This systematic review by Oner et al. (2020) provides valuable insights into the use of NSIs
for evaluating nursing care quality. The review highlights the importance of considering all
four categories of NSIs – organizational-focused structure, nursing-focused
processes/interventions, nurse-focused outcomes, and patient-focused outcomes – to get a
comprehensive picture of nursing care quality. The findings also suggest that nurse staffing
levels, patient mortality rates, and nosocomial infection rates are some of the most critical
NSIs for monitoring and improving nursing care quality.

The comprehensiveness of this review by Oner et al. (2020) makes it a valuable resource for
researchers, healthcare organizations, and policymakers who are interested in improving the
quality of nursing care.

Craig et al
Summary of "Developing and evaluating complex interventions:
the new Medical Research Council guidance"

The article by Craig et al. discusses the complexities involved in developing and evaluating
complex interventions in healthcare and public health, presenting the updated guidance from
the Medical Research Council (MRC).

Background and Importance of Complex Interventions

 Complex interventions are widely used in health services, public health
practice, and areas of social policy with health impacts, such as education,
transport, and housing.
 Evaluating these interventions presents unique challenges beyond those
faced in simpler evaluations.
 In 2000, the MRC provided a framework to assist researchers and funders
in adopting appropriate evaluation methods, which became influential and
widely cited.

Need for Updated Guidance

 Since the 2000 framework, significant experience has been gained with
both conventional and innovative evaluation methods.
