Does she have any other symptoms or concerns we should discuss On what part of her body did the rash start and where did it spread: Has she been having fevers: yes she has When did her fever start: 3 days ago Did she have any other problems two to four days before the rash started What treatments has she had for her rash? Does she have any allergies Is she taking any over the counter or herbal medications Is she coughing up any sputum How high is her fever Is her rash painful or itchy What treatments has she had for her cough How many stool diapers does your child have per day How is her appetite? Any recent changes; 1 PMH question PHYSICAL EXAM SKIN: warm and dry, maculopapular pink rash on the abdomen and extremities HEENT: inspect eyes no ptosis erythema or swelling, conjunctiva pink no discharge Examine pupils, inspect ears, look in ears Inspect nose, look up nostrils, inspect mouth/pharynx Chest Wall & Lungs Visual inspection anterior posterior chest Normal lung sounds Normal heart sounds ABDOMEN Flat symmetric with no scars No pain, tenderness, masses or pulsation abdomen palpation. No guarding, rebound tenderness EXTREMITIS Visual inspect all extremities Palpate extremities
