Patients with anticoagulation mediation Patients with anticoagulation therapy are at risk of developing bleeding. Although tooth scaling and the planning procedure are non-urgent dental care for anticoagulated patients, those willing to undergo it can do so. However, the following protocols should be followed. Patient's assessment. The reason for starting Warfarin Therapy is indicated. The duration of the therapy is determined. The frequency of monitoring the treatment is identified until its stability is achieved. Evaluation is done to determine any local factors that may increase any form of bleeding. Suppose the patient was before the procedure on a short course of Warfarin therapy for less than six months. In that case, the tooth scaling and planing procedure are delayed until the patient completes the warfarin Therapy. The INR levels are used to determine the safety profile for the procedure. In the case the INR ranges between 1.5 to 3, the procedure should commence. However, the patient is referred to a physician for Warfarin Therapy if the INR is greater than 3. harryson 3 Proper endocarditis Prophylaxis is done using a single dose of amoxicillin where possible. NSAIDs are avoided to reduce the risk of bleeding. Tylenol can be to act as a short pain reliever (Jover et al., 2007). Patients with a Blood Pressure reading of 200/140 A patient with a blood pressure of 200/140 is hypertensive. The patient is at stage 2 hypertension at this stage, as indicated by the blood pressure levels. The elevated blood pressure is known to cause excessive intraoperative bleeding during surgical procedures. However, under controlled blood pressure of below 180/90, Tooth scaling and planning can be done. The following protocols are followed before the dental practice. Firstly, the blood pressure should be controlled. In the case whereby the procedure is not urgent, the blood pressures should be managed. The patient is referred to the physician to take medication to lower the blood pressure. On the day of the procedure on successful control of blood pressure, the patient is encouraged to take their medication. The process should be carried during morning hours or late evening. In the case of anxiety among the patient, anxiolytic agents are administered to the patients. A 5-10 mg of Diazepam is given to the patient the night before and 1-2 hours before the visit. Anesthesia is done with Mepivacaine, which has no vasoconstrictor. Analgesics and antibiotics are used to counter any instances of increased BP. As discussed earlier, the NSAIDs should be avoided. In the case where the blood pressure fails to lower, Furosemide 40 mg is administered orally. Failure in blood reduction, captopril 25mg is administered through the oral route. Sudden body movement should be avoided during the procedure since they may cause orthostatic hypotension.
