Describe the steps of a dermatome screen
Patient sits with their hands on their lap, palms up, eyes closed, deliberately "swipe"
over an area corresponding to a specific nerve root, compare sides and grade
Where would you swipe to test C5?
Mid-deltoid
Where would you swipe to test C6?
Thumb and radial aspect of index finger
Where would you swipe to test C7?
Ventral aspect of middle finger
Where would you swipe to test C8?
Ulnar border of ring/5th finger PINKY
Where would you swipe to test T1?
Medial forearm
What is a myotome screen?
Resistive test that corresponds to spinal nerve root innervation AND peripheral nerve!
Tests motor function
If a nerve root is pinched or there is a lesion how is the muscle effected?
The muscle it corresponds to will be weak (the corresponding area on the skin will also
lack sensation)
How would you preform a myotome assessment evaluating C5?
Resisted should abduction/elbow flexion! Both arms in a broken "T"/scarecrow position
and pushing down evenly on both sides OR patient makes a muscle, you pull wrist back
towards you while they try to pull towards them
How would you preform a myotome assessment evaluating C6?
Resisted wrist extension/elbow flexion! Patient makes a fist and tries to resist as you
push down on it towards the ground OR make a muscle, pull wrist back towards you
while they try to pull towards them
How would you preform a myotome assessment evaluating C7?
Resisted elbow extension! Patient makes a muscle, you push their arm towards them
while they try to extend it out towards you
How would you preform a myotome assessment evaluating C8?
Resisted finger flexion/thumb extension! Put 2 fingers out and have patient wrap around
and squeeze OR have them do a thumbs up and resist while you push down on the
thumb
