Health and social care professionals help individual by offering them information,
advice, support, care and treatment to help those with dementia and their carers.
A GP (general practitioner) is a family doctor and they will be the first point of call for
Robert and any concerns he has or his wife has about his general health. The GP
will have an important and growing role within the assessment, treatment and
support for patients and will help to provide support for rheir family. If the patient is
worried about their memory, they will need to contact the GP. If dementia is
diagnosed then the GP will refer the patient to the consultant doctor or to a memory
clinic. The GP will also provide them and their family with information on any helpful
services or will refer them to another professional within health or social care.

There are many different nurses who can help and individual with dementia.
The district nurse of the community nurse is members of the primary care team.
They will have had some extra training and will provide advice and care for the
patient and their family at home. They can help the patient to take their medication,
give them injections, check their blood pressure and clean or dress a wound.

The community mental health nurse will help to provide treatment, care and support
for the client regarding their dementia. They will carry out an assessment of the
patient at their home and will advise them and their family and any carers in ways in
which they can cope and ways to improve their quality of life. The client will need to
get a referral from the GP or the consultant to see the community mental health
nurse.

There are also other health professionals that they can see who are known as “allied
health professionals” this can be in your home, care home or in the hospital.
The occupational therapist will offer them advice on the way in which they can
maintain the ability and independence in their daily activities for as long as they can
continue them. This could include providing them information on assistive technology
if they are experiencing cognitive problems and adaptions and equipment for the
home which can help the client if they are having mobility or physical difficulties.
Physiotherapists will advise the client on their exercise and mobility throughout the
stages of dementia. They will also advise them on how to reduce the risk of falling
and on the correct positioning for those who are having advanced mobility problems.
They will also advise their family and any carers into the safe way of helping them to
move or exercise.

A speech and language therapist will advise the and their carer or family on the
better ways to communicate and will also help with any swallowing difficulties they
might be having. They can teach them new exercises which can help to strengthen
the muscles which are used in speech and swallowing and will advise them on a
