P2- Identify the processes and assessment tools involved in planning support for individuals with
different needs who use social services.

M2- Describe how three key professionals could be involved in planning support for individuals.

D2- Assess potential issues which could arise from the involvement of several professionals in the
planning of support for individuals.

All the needs of the service user must be looked at when a care plan is being constructed, it must
also aim to meet every need that the individual has. An assessment will take place then the needs of
the service user will be established, but not all needs may be identified right away. This is the reason
for a number of dates when the care plan is reviewed and altered.

A care plan is put together once the individual has been referred by one of the following;
 Themselves
 A family member
 Social worker
 Support worker
 General practitioner
Following the referral, a holistic assessment will be carried out by a manager, this will help to
establish the needs of the service user. This may include the individual needing to receive meals to
their home. Each need will be noted down, it will include what roles and responsibilities each health
professional has. The progress of the care plan must be recorded, as well as noting down any
alterations that should be made. Everybody involved in the care plan must be notified when any
changes are made to the plan. The staff members must also demonstrate that they are aware of the
procedures that are involved in the care plan. They must then sign this as proof. The carers will be
observed by members of management, who will then set new targets once old targets are complete.

Assessment tools may be used when carrying out when caring for an individual, this may involve;
medical history, checklists, observations and diaries. Each of these tools will allow the health
professional to provide holistic care to the service user, this will ensure that all of their needs are
met.

a diary that is kept by the service user could help to identify any likes, dislikes, requirements, desired
and their hobbies. As well as it possibly sharing what they think on the care that they are currently
receiving. It may also include any routines that the individual may have, this may include what they
like to do before they sleep and what they like to do once they wake up. The service user’s medical
history may also be looked at as this can help to gain information on the individual, such as;
 Height
 Weight
 Blood pressure
 Blood sugar levels
 Pulse
These factors will be regularly monitored to determine if the service user is making progress or not.
A checklist may be another assessment tool to identify particular requirements that the patient may
have, these will be looked at by care manager or social worker.

Questions may also be used to assess the individual, this will help gain a better understanding of the
circumstances that the patient may be facing. It may include a range of closed questions and open
questions as leading questions should be avoided as this may look like the misuse of power. The
recording of incidents is also used as this can help to decide if support is needed or not.
