Including one credit L&R and three on S&S. Four credit hours to be w/in the 3 years prior to expiration. Valid for only one license renewal period. Continuing ed. credits are required as of August 1, 2014. Knowledge & skills up to date: instructor 45 hours of continuing ed. w/in 3 years before license expiration. Including at least 15 hrs. of teaching related material & 15 hrs. related to analysis & use of prof. clinical products. To renew instructor license, instructor must have an active operator or salon manager license. Knowledge & skills up to date: school manager Must maintain active salon manager license, & documentation of having practiced in licensed school as school manager for at least 1,800 hrs at any time w/in the 3 years prior to expiration; or 400 hrs via regular weekly experience w/in each of the last three years; or successful refresher course completion of at least 40 hrs, w/in the 3 years prior to expiration or reactivation Procedure for lapsed license If expired, then license reissued after submission of request for renewal, proof of experience & education; & payment of fees. If more than 3 years have elapsed, then apply for new license Procedure for inactive license If expired by < 3 years, then active license reissued for remainder of licensing period. Make application in writing and on all forms provided by the board, 3. Pay the fees identified in MN statutes, 4. Meet the requirements for initial licensure, 5. Meet requirements for individual license, for license renewal -- practical educational requirements -- and requirements for reactivation -- refresher courses, & 6. At least 2 yrs expired from the effective date of revocation Transfer of license prohibited Licenses not transferable and to be used only by the named licensee Salon application for licensure Apply in writing to the board on forms supplied by board: A. Name & address of salon, names, addresses & phone numbers of owners B. Notarized signatures of owners C. Days & hours of operation D. Evidence of compliance w/ zoning, ordinances & fire codes E. Floor plan to scale on 8 1/2 by 11 or 8 1/2 by 14 in paper. Noting entrances, exits, location & dimensions of all required areas, facilities, & equipment Maintaining a salon license A. Continuously comply w/ all L&R B. Renew license before expiration C. Display required documents Salon license renewal All licenses expire on last day of month of initial licensure of the year due. Each licensee is responsible for renewal. Salon license not renewed by the last day of the month of initial licensure of the year due is considered unlicensed as of the first day of the following month. Not receiving notice from board does not constitute an excuse. Licensee shall pay fees before expiration of current license Licensee shall state name, address, & phone number of salon owner on renewal application Delinquent salon licenses -- failure to renew Failure to renew a license before expiration shall result in delinquent salon license. This license must be renewed w/in 30 days Delinquent salon licenses -- renewal If <30 days since expiration, then submit written request for renewal and fees If >30 days since expiration, then salon ceases operation until new application is submitted and license issued Salon requirements -- location No cosmo service shall be provided in a place other than a licensed cosmo salon, nail salon, esthiology salon, cosmo school Salon requirements -- license No salon shall operate w/out a license Employment of unlicensed person No salon shall employ or allow unlicensed persons to perform any cosmetology service Salon requirements -- termination of license License is not permanently transferable & terminates after occurrence of change of either ownership OR location. How many days upon change of ownership or location does owner have to apply and receive new license? 60 days Change of name Owner shall inform board in writing w/in 60 days of name change, including old & new name, and pay fees. License shall be issued in new name for remaining term of old license. Old license returned to board upon receipt of license in new name. Physical requirements -- space At least 120 square feet of work space exclusive of restroom, reception or supply area for one-practitioner cosmetology salon, 110 for a one-practitioner esthetician salon, and 100 for a one-practitioner nail salon. Physical requirements -- plumbing, water supply Clean, hot, and sanitary running water provided in work area, dispensary, and toilets. Each salon shall provide toilets on premises or in adjacent common area. Toilet, clean and sanitary; shall contain, at minimum, a commode, lavatory, soap dispenser, single service sanitary towels, and waste basket. Door kept closed at all times. Physical requirements -- ventilation Salon ventilation shall comply w/ local building codes and ordinances Fixtures & furniture Separate work station w/ chair, storage & mirror per licensee on duty Fixtures & furniture All shampoos given in shampoo bowl w/ plumbing including hot & cold water Fixtures & furniture All furniture shall have washable covering or finish, kept clean and in good repair at all times Fixtures & furniture One waste receptacle at each work station. Emptied at least daily. Lined with disposable bag or washed daily. At least one container exclusively for soiled towels and linen. Kept closed at all times & comply w/ fire codes. At least one large covered container for disposal of garbage. Emptied at intervals. Lined with disposable plastic bag or washed daily & comply w/ fire codes. Fixtures & furniture One storage cabinet or container for all clean linen & towels equipped w/ tight fitting closable doors or covers which shall be kept closed Fixtures & furniture Storage space for all supplies. Supplies containing harmful material shall be cospicuously labeled and kept inaccessible to clients Fixtures & furniture Maintain readily accessible first aid supplies Fixtures & furniture At least one readily accessible fire extinguisher.
