HPI: A female, 28 years old comes to the office and reports unusual discharge from the vagina, pain and irritation when passing urine, aggravating pain in the lower belly and an increase in the frequency of passing urine. She sates that the onset of pain and irritation when passing urine was 2 days ago. She adds that the discharge from her vagina has a bad smell and its onset was over a week ago. The onset of the pain in her lower belly was7 days ago and has been getting worse with days. Patient admits to having unprotected sexual intercourse few weeks ago after which she started having discharge from her vagina with bad smell. PMH: Medication History: Patient is not under any medication and she is intolerant to Trimethoprim which causes her to develop rashes when she takes it. Patient denies discharge from the breasts and adds she has a pap smear six months ago. Other illnesses: Chlamydia and gonorrhea and Urinary tract infections that have been recurring for the past three years. Allergies: None. Surgical History/ OB/GYN: female sterilization 24 months ago.
