Inleiding

1. Kenmerken van de schildklier
- Rijkelijk gevasculariseerd, bestaande uit 2 lobben & 1 isthmus
- Start aanleg: 12w ZWS
- Histologie
o Follikelcel/thyrocyt
▪ = bolvormige follikels met éénlagig epitheel
▪ Centraal: visceus & eiwitrijk colloïd
• = stockageplaats voor schildklierhormoon
o Parafolliculaire cellen (= C-cellen)
▪ Tussen follikels
▪ Productie van calcitonine

2. Schildklierhormoon
- Synthese
o Uitgaande van jodium
▪ Opname via actieve jodiumpomp = natrium-iodide symporter (NIS)
• Tegen concentratiegradiënt in
o Uitgaande van eiwit thyroglobuline (Tg)
▪ Synthese in thyrocyt zelf
o Enzymatische processen
▪ O.a. gemedieerd door thyroperoxidase (TPO)
▪ Tyrosineresidu’s gejodeerd op thyroglobuline
• Koppeling in colloïd
- Secretie
o Onder vorm van thyroxine (T4) of trijodothyronine (T3)
▪ 80% T4, 20% T3
▪ T4 = prohormoon
• Lage affiniteit voor R
• Via dejodinase: metabolisatie tot o.a. actief T3
▪ T3 = actief hormoon
o Serumconcentratie T4: 60-70 x hoger dan T3
▪ Meer aanmaak, langere halfwaardetijd
- Transport
o > 99%: eiwitgebonden
▪ TBG, prealbumine, albumine
▪ = totaal T4
o < 1%: vrij T4
- Rol
o Effecten op vrijwel alle weefsels
o In vroege leven: essentieel voor normale neurologische ontwikkeling
▪ Bv. migratie & differentiatie van neuronen
o In latere leven: cruciaal in regulatie van:
▪ Basaal metabolisme: thermogenese, calorigenese

▪ Intermediair metabolisme
• Eiwitten: bv. normale spierfunctie
• KH: bv. gluconeogenese Vetten: bv. lipolyse
o Betrokken bij processen als lengtegroei, gonadale functie…
- Productie & secretie onder controle van CZS
o Door negatieve feedbackregulatie
▪ Via hypothalamus: TRH (thyroid releasing hormoon)
▪ Via hypofyse: TSH (thyroid stimulerend hormoon)
o Omgekeerd logaritmisch-lineair verband tussen serum TSH & serum T4
▪ Bv. halvering serumconcentratie T4 → > 100-voudige toename van TSH
- Autoregulatiesysteem
o Door jodiumstatus
▪ Jodiumdeficiëntie: ↑ TSH-gevoeligheid + ↑ NIS-expressie
• = preventie lage schildklierhormoonspiegels
▪ Jodiumexcess: ↓ TSH-gevoeligheid + tijdelijke inhibitie jodiuminflux
• = Wolff-Chaikoff effect
• = preventie hoge schildklierhormoonspiegels
- Zwangerschap
o ↑ hCG
▪ Stimulatie thyrocyten
▪ 7w ZS tot einde 1ste trimester: compensatoire laag serum TSH + ↑ totale
T4- & T3-serumwaarden
• Gevolg: ↑ schildklierhormoonproductie (+ 50%) + ↑ volume
schildklier (10-15%)
• Doel: foetus van voldoende materneel schildklierhormoon
voorzien
o = cruciaal in 1ste helft ZWS → foetale
schildklierhormoonproductie = absoluut of relatief
ontoereikend
o ↑ TBG (= thyroidbindend globuline)
▪ ↑ extrathyroïdale serumpool van T4

Hoofdstuk 1: Exploratie van schildklierziekten

1. Klinisch onderzoek van schildklier
- Inspectie halsstreek
- Palpatie
o Volume (nl: 10-20 mL)
o Diffuus of nodulair voorkomen
o Consistentie
▪ Week/hard Pijnlijk/pijnloos
o Ligging
▪ Duikend achter sternum-clavicula?
o Doorbloeding
▪ Thrill bij palpatie / continu geruis bij auscultatie → hypervascularisatie
o Druk op omliggende halsstructuren
▪ Bloedvaten: vena cava superior compressie
▪ N. recurrens: heesheid
▪ Trachea: stridor
▪ Slokdarm: dysfagie voor vast > vloeibaar voedsel
o Vergroeiing met huid
o Adenopathieën
▪ Week/hard Pijnlijk/pijnloos

2. Technische onderzoeken
a. FUNCTIONELE ONDERZOEKEN
i. Labo
- TSH
o Indicatie: screening schildklierdysfunctie
o Hypothyroïdie
▪ ↑ TSH
o Hyperthyroïdie
▪ ↓ TSH
o Subklinische schildklierdysfunctie
▪ Gestoorde TSH met normale of gecompenseerde
schildklierhormoonconcentratie
o Hypothalamische/hypofysaire insufficiëntie
▪ Schildklierhormoonconcentratie ↓
▪ Met TSH = laag of inadequaat normaal
o 1ste keuze bij screening schildklierdysfunctie
o Normale populatierange voor serum TSH: 0,4-4,5 mU/L
▪ Zwangerschap: normale ondergrens TSH 0
o Beïnvloeding TSH:
▪ Diurne variatie
▪ BMI (↑ TSH bij ↑ BMI)
▪ Leeftijd (↑ TSH bij ↑ leeftijd)
▪ Bepaalde geneesmiddelen
