NR 501 Week 5: Analysis and Application of a Nursing Model Select either the Patient-Centred Nursing Framework or the Chamberlain Care Model. Consider how the following components would be included in an analysis of the selected model: • Origin of the model - • Meaning of the model – • Logical adequacy of the model - • Usefulness of the model - • Generalizability of the model – • The degree of parsimony within the model - • Testability of the model – • What rationale can you provide which validates the selected model as a theoretical framework for nursing practice? Be sure to include scholarly references to support your discussion. Introduction Person-Centered Nursing (PCN) is the health policy and practice with a main principle focuses on the idea that the patient or the person as the center within the delivery of care. Also, McCance & McCormak (2011) defined concept of person-centered nursing as the application of holistic approach to nursing practice that was created through therapeutic relationship between the patient and all healthcare providers. It includes patient and care provider mutual respect and understanding, and the right to self-determination as well as being empowered by culture that promotes nursing practice development and improvement. Origin of the model The origin of this nursing model was started by Carl Rogers. Carl Rogers is also known as the “father of psychotherapy research” (Fazio, Pace, Flinner, & Kallmyer, 2018, para. 1). It is also noted in this article that in the late 1930’s Rogers works as clinical child psychologist which helps develop the PCN nursing model and later improved. He also highlights the foundation of healing effect which is based on the life experiences of every person. Moreover, in 1988, Tom Kitwood was the first person to use the term Person-Centered Nursing care who examined and showed the concept of Person-Centered Nursing which focuses on relationship and communication (Fazio et al., 2018). Meaning of the model The meaning of the PCN is the holistic approach of nursing care delivery that focuses on the patients’ healthcare issues and needs rather than the needs of the practitioners or healthcare organization. This means that the patient is the center of the healthcare delivery system. The priority of person-centered nursing (PCN) care is the patient safety and being aware and meeting the patient healthcare needs. Person-centered nursing also means being culturally and spiritually competent. According to Fazio et al., (2018), being culturally and spiritually competent is not just about religious belief but also characterized by moral and values, therapeutic relationship and self-expression that promotes the sense of acknowledging the importance of patient’s personal system. Although there are numbers of meanings of PCN, all meaning promotes personhood, independence, commitment and health care delivery empowerment based on the patient preference and needs. Caregivers that uses the model of person-centered frameworks recognizes that each person is unique, and a personalized care plan is vital to promote positive health outcome. The emphasis of PCN care is to get to know the person within the patient itself, to effectively connect with the patient, and listens to the patient and treat as a person. Logical adequacy of the model According to Cingel et al., (2016), there are several studies and research were completed to interpret and improve the concept of person-centered nursing (PCN) model. One study shows that the need for PCN care knowledge is conveyed by nurses and healthcare organization. Nurses and other caregivers wish to know the proper use of person-centered nursing care in more details especially when there is an issue such as increased work load, or non-compliance patient. Cingel et al., (2016) noted that nurses and other health care provider have inadequate knowledge on how to accomplish PCN care due to the lack of transparency about what PCN care means among healthcare providers, even though there is an increased awareness of PCN model in healthcare. Therefore, the PCN care model is at risk to fall down to the same level of inadequacy from the last time it was created. Usefulness of the model Health care system will use this model to create a program that will help in meeting the patient’s needs and will promote collaboration among staff as well as empowering the organizations, communities and patients. In addition, continuity of care can be accomplished through community collaboration and identifying resources needed by patient post hospitalization such as coordinating home health care services or outpatient care services (Santana, Manalili, Jolley, Zelinsky, & Lu, 2018). The environment of modern nursing practice is changing along with the acknowledgement of PCN care model within the healthcare field. According to Santana et al., (2018) the increased number of studies suggest that when caregivers and healthcare organization incorporate or use PCN care model in their practices it resulted in the increase positive health outcome, as well as an increased patient satisfaction. In addition, an increased patient treatment compliance, decreased in mortality rates, decreased length of stay in the hospital as well as decreased hospital acquired infection is correlated to the hospitals use of person-centered nursing care model. Generalizability of the model The patient or person shapes the person-centered nursing model and act as the essential part of the care delivery system. This nursing model must be used in a flexible manner to accommodate all the patient needs in regard to patient’s culture, values and beliefs. According to Santana et al., (2018) a result of a positive change in health is accomplished through the presence of person-centered nursing care approach within a multicultural patient or family. The core principle of person-centered nursing care model is the most important ingredient that makes it useful worldwide. According to Joseph & Murphy, (2012), PCN model was translated to twelve (12) different languages and is useful and successful throughout thirty (30) countries. This model promotes a unified PCN care delivery that can be used in a systematic level. Joseph & Murphy, (2012) proposed an integrated PCN care model which encourages interprofessional collaboration and interaction with the client. In addition, they aim to introduce different interventions and measures to develop new healthcare policies and practices that will cultivates a strong support towards the use of PCN in healthcare setting, The degree of parsimony within the model According to Santana et al., (2018) person centered nursing model provide structure that helps provide reduced and fast healthcare access. Also, it is often used to help patient secure preferred healthcare treatment and providers to improve patient’s health outcomes. PCN improves providers referrals, procedures wait time, and increased health care provider availability. The positive effect of PCN to health care system is listed by Santana et al., (2018) such as improved hospital services utilization, limits the number of hospital readmission, decreased morbidity and mortality rate for both chronic and acutely ill client. Testability of the model A survey conducted about person-centered nursing care using the patient point of view. Ten (10) patients were surveyed and interviewed by Sharp, McAllister, & Broadbent, (2015) seeking to get to the bottom of patient experience with person-centered nursing care model. The result shows that holistic and competent nursing care approach correlates to the use of PCN care which leads to improved patient positive outlook on recovery from illness and improved therapeutic relationship between the caregiver and the patient. Also, in the study Utilizing a qualitative design encompassing observation and semi-structured interviews conducted by Sharp, McAllister, & Broadbent, (2015) shows that under the philosophy of person-centered care model, patient collaborates effectively with their treatment and caregivers in the acute care setting. Also, other findings show the importance of effective communication on person centered nursing care because it largely influences the nursing practice which promotes patient safety. What rationale can you provide which validates the selected model as a theoretical framework for nursing practice? Be sure to include scholarly references to support your discussion. A good example of person-centered nursing care is the planet tree approach of care. this approach is noted to be helpful in providing efficient patient services and supports the continuum delivery of nursing care and healthcare services. According to Santana et al., (2018) the newly created evidenced-based practice, provides crucial framework that incorporates the person-centered nursing care model. It includes the best nursing practices that allows patient perspective or point of view to be respected and reflected by the health care provider and by the health care system. Moreover, Santana et al., (2018) identified this framework as highly effective in providing the best quality of nursing care and can be easily used and adapted to various health care settings. Instructor response Hi Dr Hartshorn Thank you for your question. For me, I think that understanding the meaning of person-centered nursing (PCN) model is very important to be able to act or put PCN into daily practice. There are several meanings of PCN such as independence, person hood, commitment and health care delivery empowerment that follows patient preference and needs. In addition, recognizing that each individual is unique, and creating apersonalized care plan suggest PCN use which is vital in promoting positive health outcome. The emphasis of PCN care is to get to know the person within the patient itself, to effectively connect with the patient, and listens to the patient and treat as a person. Furthermore, Cingel et al., (2016) noted that nurses and other health care provider have inadequate knowledge on how to accomplish PCN care due to the lack of transparency about what PCN care means among healthcare providers, even though there is an increased awareness of PCN model in healthcare. To answer your question, I think that one best solution to help other staff understand and incorporate PCN in their daily practice is to implement the use of PCN model in the clinical area and allow staff to participate in the PCN care model evaluation process (which will promote transparency). International Practice Development Journal, 6(2), 1-17. Student Response Hello Stephanie, You did great on your discussion about person-centered nursing (PCN) framework. In our lesson this week, it indicates that PCN is focused on the patient health care needs, rather than the need of the organizations services being provided to patient. This means that PCN encourages patients involvement on their care by respecting and allowing patient to choose their care preferences. The PCN focuses on patient safety, needs, and comfort. In addition, PCN provides holistic nursing care approach, which embodies the profession of nursing. Holistic nursing care is about treating patient as a whole person. This means that the nurse looks at the patient as a person not just as a patient. Holistic nursing care includes treating patients mentally, physically, spiritually as well culturally not just the disease it self. I agree with you that PCN is supported by empirical studies. According to Cingel et al., (2016) there were several research studies to evaluate PCN's effectiveness and ways to improve its usefulness in the clinical setting. The person-centered approach is shaped by making the individual the essential part of the treatment. Regardless of culture, this approach must be flexible to accommodate client health needs.
