, 1) When Injuries and assaults occur before admission, the correctional officer
must ensure that a validation in this regard must be made on the J94 body
receipt, SAP 216, receipted copy of the warrant, and a similar receipt when an
inmate is admitted to a prison in an injured condition, or where it is alleged on
admission that an inmate was injured or assaulted before admission. A
detailed description of Mr A’s condition must be recorded on a computer and
in the medical history chart or file, including all visible injuries and marks, as
quickly as possible, and the correctional officer needs to ensure that Mr A has
a medical check-up soon after.

With regards to Mr A’s case, I the correctional officer, would prepare the
medical reports for personal use, and any other reports for the South African
Police Service (SAPS) will be completed and sent straight to them. I will report
the matter to the local police station as they may be a chance of prosecution
for any suspects that may be identified. If the claims are made against the
SAPS, I will notify the local district area manager.

If the convict requires immediate medical assistance and the attending
physician is unavailable, the injured or ill individual must be referred to a
public hospital for treatment. When an offender leaves the prison for a short
period of time and then returns with an injury, the same procedure applies.

2) Any person, whether guilty or not, may not be admitted to a correctional
facility unless a warrant or written order issued by a court or other authorised
person can be provided. The warrant or order must be addressed to the
appropriate correctional facility and kept safe for the duration of the inmate's
confinement. The head of the correctional centre, or delegate, is responsible
for ensuring that the terms of any legal warrants are followed to the letter. The
state, or any relevant member or temporary member of the Department of
Correctional Services, will not be accountable for any actions that may occur
because of any such warrant being issued in an irregular manner. Every
person who is lawfully incarcerated should be kept in safe custody until he or
she is lawfully released or removed, according to the head of the correctional
centre or his or her delegate.


3) 3.1.1 Admission record

All of Mr A’s personal information, as well as a complete description of his
property and the registration number assigned to him, must be input into a
computer. To validate the accuracy of the information, both Mr A and the
receptionist must sign the report. The warrant's sentences should be written
out precise and accurately. It's important to keep track of any prior convictions
