Mental Health Case: Andrew Davis Guided Reflection Questions Opening Questions 1. How did the simulated experience of Andrew Davis’ case make you feel? This Vsim with Mr. Davis made me feel more empathetic toward alcoholics. It provided insight that everyone is just going through life in their own way. It helped me fell confident that I may be of some use going forward if I find myself dealing with a patient going through withdrawl from alcohol. 2. Talk about what went well in the scenario. I feel the timing in regard to my assesment, education, and interventions was good. It made it feel much more realistic. Incorporating my knowledge base from lecture showed to be helpful while choosing an answer for Mr. Davis. 3. Reflecting on Andrew Davis’ case, were there any actions you would do differently? Ifso, what were these actions and why? If I could go back, I would choose a better time to provide medication education. Also I would change the order at which I obtained vital signs and asked question regarding the CIWA. Scenario Analysis Questions* 4. What issues have you identified that could be barriersto a successful treatment for Andrew Davis? I believe the issue Mr. Davis has with his wife and daughters will prove to be a significant barrier to a successful treatment. 5. How would you address the denial? To address the denial, as a nurse I would promote understanding of the patient as well as awareness of their needs. In theory this should allow the patient to become more comfortable in addressing their reality. 6. Identify a support group that would be beneficial to Andrew Davis related to his position in the community. Mr. Davis should attend an A.A. group. These deal with pastorsfrom many faiths often. This will also give flexibility to his schedule. 7. Identify support groups that would be beneficial to Andrew Davis’ family. Mr. Davis family would benefit greatly from an Al-non group. 8. Andrew Davis is currently in Stage I of alcohol withdrawal. As he moves into Stage II (24 to 72 hours) and then into Stage III (after 72 hours), what behaviors should be assessed for and what safety measures should be initiated? As Mr. Davis moves through the stages of alcohol withdrawl, it would be wise to monitor his vitals, asses for tremors, monitor his mood, assess for halucinations, assessfor photosensitivity and audio disturbances, place him on seizure precautions, and maintain a safe and hazard free environment. Concluding Questions 9. How would you apply the skills and knowledge gained in the Andrew Davis case to an actual patientsituation in different acute care units(emergency room, intensive care unit, obstetrics unit, etc.)? I can apply this new knowledge in many different areasthroughout medicine. In the ER I can now save time and identify patients who are experiencing alcohol withdrawl faster. Also, in an internal medicine clinic I can identify patients or family members in need of community groups. Finally in an in-patient rehabilitation clinic I can establish better therapeutic relationships with patients.
