Dena stated the most common infections she has seen readmission for in our facility are pneumonia and UTIs. Our elderly population seems to be the most susceptible to readmission as they tend to take longer to recover. Management has attempted to intervene and help with HAIs by implementing tools and technological support, but it has not had much of an effect on the number of readmissions. These hospitalized patients reveal an imperative need to do something other than sticking to the standard precautions. Some examples of standard precautions include but are not limited to cough protocol, good hand hygiene, using personal protective equipment, following safe injection practices and disinfecting contaminated material and/or surfaces. Dena also brought up how she feels that there is still room for improvement with interdisciplinary collaboration as she feels that management needs to clarify the roles of other department members. Interdisciplinary collaboration should include nursing staff, physicians, pharmacy staff, physiotherapy staff, laboratory staff, clinicians, and other stakeholders who may be helpful in the reduction of HAIs. One or two members from each of the departments should be selected to participate in quality improvement initiatives to seek a reduction in the prevalence of HAIs. Identifying Issues During my interview with Dena we discussed many issues which we both felt needed more attention but the one we both felt should be discussed further was hospital acquired infections and how there is a need for more interdisciplinary collaboration. On the Med/Surg unit, hospital acquired infections are becoming more common as the patient census continues to rise. Interprofessional collaborations involving nursing, physicians, PT, Lab, and pharmacy would allow them to examine the subject together and come up with a logical resolution. By means of implementing an interdisciplinary approach will allow the care team to learn from each other’s scope of practice, skills, experience and knowledge. This could help tremendously with the prevention of hospital acquired infections as new measures can be put in place. Change theory that could help develop an interdisciplinary solution The implementation of change in healthcare facilities can be intimidating task. People are resistant to change it is a fact. When a new policy is put into place there will always be those who have a hard time adapting to the new change even if it is for the better. There are theories and models that can provide a type of outline on how to give support for the adaptation to the change in policy. Change management models and theories can predict the change process and provide a good way to manage the process. The Plan-Do-Study-Act Model (PDSA) is often used in healthcare settings to boost methods.
