The issue within our physician practice has been identified after talking with our office
manager, nurse manager, and our director. The problem that I am seeking to solve within our
physician practice is related to ineffective communication among the members of the healthcare
team. This proposal is related to education in communication while creating a process for
precise, accurate, and effective communication.
A1a. Explanation of Problem or Issue
The daily function of a physician practice relies on communication. Not only is
communication required but it needs to be effective. Modes of communication includes but not
limited to, written, verbal, face-to-face, and electronic. Communication not only needs to be
effective when speaking to our patients but communication also has to be effective when we are
collaborating with other healthcare members within our practice. If communication is adequate
and effective then this reduces the chances of any medical errors. The practice where I am
employed has had questions and issues when it comes to communication and making sure the
right information gets through to the right person so our patients are adequately cared for.
A2. Investigation
The problem of ineffective communication has been thoroughly investigated within our
practice. After reading further, you will see that I have also reviewed information from different
professional journals related to communication in healthcare, and it has proved that
communication is a problem healthcare. I met with my office manager, nurse manager, and
director to review the different issues in our practice and communication seems to be the biggest
issue in our practice. I also met with the physicians in the practice to discuss what could be
improved inside the practice, and they also noted that communication between all staff could be
improved. During my investigation I also asked the nurse, scheduler, and receptionist of
