CARDIOVASCULAR ASSESSMENT General Survey: E.D. is a 33 y. o. married with 4 boys, slender, black female nursing student who is in for “cardiovascular check-up”. Well groomed, dressed appropriately. Cooperative during exam. Alert and well-oriented to person, place, and time. History: Denies any cardiovascular complaints such as, chest pain, or difficulty breathing. Patient reports having a history of being hypertensive. Surgeries: Patient had a hysterectomy. Dermoid tumor resulted in right ovary removal. Family History: Mother and father have a history of hypertension, brother and uncle have asthma. No family history of heart attacks. Social History: Denies the use of Tobacco, occasionally drinks wine. Current medications: Lisinopril 100mg, Labetalol 100mg, Allegra 200mg, Xanax 20mg, Celaxa 30mg Allergies: Sulfa Drugs Chief complaint (if applicable): No chief complaint
