3.1 Inleiding
Diagnose fase: is bedoeld om de gezondheidstoestand van de zorgvrager en de bijbehorende
problemen vast te stellen en te beschrijven.
 Verpleegkundige beoordeelt alle verzamelde gegevens kritisch, zodat ze een op onderzoek
gebaseerde conclusie (verpleegkundige diagnose) kan trekken




Diagnose is zowel proces als een product
1. Diagnose is de naam van een fase van het verpleegkundig proces
2. Diagnosticeren (of diagnostisch redeneren) is het redeneerproces dat verpleegkundigen
gebruiken om gegevens van de zorgvrager te interpreteren
3. Het eindproduct van dit redeneerproces is een uitspraak over de gezondheidstoestand van
de zorgvrager die vaak de verpleegkundige diagnose wordt genoemd
4. Om de diagnose te stellen, hanteren verpleegkundigen vaak een gestandaardiseerde
begrippenlijst met verpleegkundige diagnosen.




Verpleegkundige diagnosen: oordelen over een reactie van een zorgvrager op een
gezondheidsprobleem
 Moet holistisch worden opgevat: niet alleen om lichamelijke reactie van de zorgvrager, maar
ook om de psychische of sociale reacties, of reacties op het gebied van spiritualiteit
