, INLEIDING

Voor de leeruitkomst bevorderen van gezond gedrag op doelgroepen niveau is er binnen het
verpleeg- verzorgingshuis gekeken naar een passende doelgroep met daarbij een
gezondheidsprobleem. Na overleg met collega’s kwam de eenzaamheid al snel naar voren als
gezondheidsprobleem.

De opdracht is verwerkt met hulp van klinisch redeneren en intervention mapping. Het is gericht op
het bevorderen van gezond gedrag bij de doelgroep. De ingezette interventies zijn opgesteld vanuit
literatuur.
