Course Description: The NDNQI Pressure Injury Training – Version 9.0 is the standardized
educational program used by healthcare organizations participating in the NDNQI database to
ensure accurate and reliable pressure injury identification, staging, and reporting. Module 2
focuses on the Knowledge Check, which tests the learner's ability to identify different wound
types (pressure injuries, diabetic foot ulcers, venous ulcers, skin tears, and incontinence-
associated dermatitis), apply the NPIAP staging system, and document findings in accordance
with NDNQI standards. Mastery of Module 2 is required before staff can participate in NDNQI
pressure injury prevalence surveys.

DOMAIN 1: PRESSURE INJURY DEFINITION & PATHOPHYSIOLOGY

Question 1

A pressure injury is best defined as:

A) Any wound that occurs in a hospitalized patient

B) Localized damage to the skin and/or underlying soft tissue, usually over a bony prominence
or related to a medical device, resulting from pressure or pressure in combination with shear

C) A surgical wound that fails to heal

D) A wound caused by moisture or friction alone

Answer: B

Explanation: A pressure injury is defined as localized damage to the skin and/or underlying soft
tissue, usually over a bony prominence or related to a medical or other device. The injury occurs
as a result of intense and/or prolonged pressure or pressure in combination with shear. The
tolerance of soft tissue for pressure and shear may also be affected by microclimate, nutrition,
perfusion, co-morbidities, and condition of the soft tissue.

Question 2

Which of the following forces contributes to the development of pressure injuries by creating a
folding or stretching of tissues?

, A) Pressure only

B) Shear

C) Friction

D) Gravity

Answer: B

Explanation: Shear is a mechanical force that acts parallel to the skin surface, causing internal
tissues to fold, stretch, and distort. Shear forces are particularly damaging when a patient slides
down in bed while the skin remains in contact with the surface. Pressure damages tissues by
compressing blood vessels, while shear adds to the damage by distorting tissue layers.

Question 3

The primary mechanism by which pressure causes tissue damage is:

A) Direct tearing of the epidermis

B) Compression of small blood vessels, hindering blood flow and nutrient supply

C) Chemical irritation from moisture

D) Bacterial invasion of the dermis

Answer: B

Explanation: Pressure damages the skin and underlying tissues by directly deforming and
damaging tissue and by compressing small blood vessels, which hinders blood flow and nutrient
supply. When pressure exceeds capillary closing pressure (approximately 32 mmHg), ischemia
occurs, leading to tissue death if pressure is not relieved.
