MSN 612 Module 4

1. Identify a time in which collaboration was not used and that led to negative
consequences for the patient. Discuss the responsibility of the APRN as a patient advocate.

An advanced practice journal article written by Steihaug et al. (2018) identifies

complications that occur from lack of interdisciplinary collaboration among advanced practice

registered nurses (APRN) and clinical physicians. This article summarizes four projects

conducted by healthcare scholars involving a rehabilitation clinic for patients over 60 years of

age who are working to go home, a young adult mental health unit, in-home rehabilitation care,

and lastly identifying the various circumstantial impediments that may occur between advanced

practice registered nurses and physicians. This qualitative Norwegian study identifies that

collaboration among APRNs, and physicians is a mandatory behavior that can have negative

consequences if not employed (Steihaug et al., 2018).

Steihaug et al. (2018) recognizes the importance of collaborative relationships among

providers when dealing with patient care. The patients from this study indicated that their needs

are not being met and at times felt rejected from choices made by the APRN. Specifically, the

young adults on the mental health unit stated that they did not feel helped or heard. The lack of

interdisciplinary collaboration among providers hampered their clinical work as the patients had

poor clinical outcomes which led them to feel apprehensive and discouraged. The study

determined that the advanced practice registered nurses were not collaborating with the

physicians and in turn making decisions in haste as the relationship between the two providers

did not have a good, solid foundation. There were noted divergences and considerations among

the two as they had developed disjointedly from one another grounded on dissimilar knowledge

bases in diverse organization and administration ideologies. Physicians often disregarded input

from the APRNs which resulted in poor collaboration and fear of rejection from the nurse

practitioners resulted in high numbers of poor patient satisfaction (Steihaug et al., 2018).

Patient advocacy is by far the most significant and important aspect of being an APRN.

Obligations to identify ailments, manage patient complications, order examinations, and

prescribe medications all play a critical role in the overall well-being of the patients under the

care of the APRN. The APRN advocates for the care of patients by supporting and preserving

their health. Patient advocacy includes communicating with the patients and their families,

educating them on the next step in their plan of care, informing them of risks and benefits of

proposed treatments, speaking on their behalf to promote safety, and establishing a professional

dyad while upholding social advocacy through interdisciplinary collaborations with the primary

care provider (Hanks et al., 2018).

2. Review the posted Collaborative/Standard Care Agreement.

Discuss the positive and negative aspects of it in the discussion

board.

The Standard Care Agreement provides advantageous and encouraging facets that will

help the APRN and physician to collaborate collectively. There will be a well-defined

understanding among both groups which permits the APRN to refer with the primary care

physician whensoever it is essential to do so. Additionally, it holds the APRN accountable in

preserving their skill sets and keeping their comprehension up to date in the area of their

authorized practice (Funk et al., 2019).

There are also some limitations to the Standard Care Agreement clause. It makes no

mention as to what the APRN is required to collaborate with the physician about. There is also

no indication as to what the APRN is required to do if the practicing physician was not working
