6. Prior to the QME examination appointment, if you did DEU
not receive the form, your office staff may contact
the Claims Administrator to inquire about it. If you do
not receive the form before your report is due to be
served on , make note of its absence in the
beginning of your report and submit to DEU anyway.
7. If the Injured Employee refuses to fill out DWC-AD 100
form , you must attach a blank copy with
the words "Refused to Fill Out" written on it to serve
on DEU with your report.
8. To determine the correct DEU office, look at the first DEU
page of the AD-DWC 101 form sent to you by the
Claims Administrator. If you did not receive the form
