Why should a PT take a history?
- Might be primary or only provider involved in case
- Drug review might be required (e.g. home health)
- ID potential problems in adherence & proper med use
- ID specific meds/changes in meds that impact rehab
- ID drug interactions or "duplicate" drugs
Sources of med information - Primary
Primary: pt
- Intro self & ask if ok to ask q's on meds
- Assess pt knowledge & engagement in managing meds
- May not be possible if pt has cognitive deficits

- Family member/caregiver
- If out-pt: Brown bag method (pt brings containers/labels of meds)
- If home health: have pt/caregiver show where meds kept
- Call pharmacy/PCP w/ pt permission
