A. Goals and Rationale
1. Explain key goals for the affiliation and specific criteria a candidate affiliate must meet.
Proven positive health outcomes through efficient and effective financial and
operational strategies.
Technological initiatives aim to effectively streamline communication
processes between lab, pharma, providers, and staff that minimize medical
errors while enhancing patient care.
The affiliate candidate has to genuinely show commitment to continuous
improvements of coordinated, high-quality, compassionate care for the
community and patients it serves, consistently researching, organizing,
managing, and growing business that is healthcare.
2. Identify three specific advantages a well-aligned affiliation would provide for TPMC.
The first advantage TPMC will gain from a well-aligned affiliation is an
extended network of clinicians made of primary care and specialist providers
that will improve access to the care needed to maintain, sustain, and
proactively treat the patient’s medical condition.
Increased quality of care due to set coordination and continuity of care that
would be implemented into every patient’s care plan.
Advancement of virtual health services and its platform. This technological
pursuit reduces onsite in-person appointments (freeing waiting area space,
reducing burnout for staff, and increasing productivity among physicians and
providers alike) and a dedicated non-emergent/non-urgent telemedicine
platform that enables a patient to be seen virtually, avoiding the hassle of
waiting on campus but in the comfort of their own home.
3. Analyze the current state of TPMC and the projected market segmentation for TPMC
anticipated from the proposed affiliation. Your analysis must be supported by
demographic data and include the following elements:
• The current state of TPMC, including anticipated market segmentation, shows
growth in our community, including but not limited to non-Hispanic Black
Americans, Native Americans, underserved, and/or patients of a fixed payment
system (Medicare and Medicaid). According to the Centers for Disease Control
and Prevention (CDC), “the prevalence of diabetes is higher in non-metro
counties in the South (14.8%) than in large fringe metro counties in the Northeast
(7.0%) (Onufrak. Et.al. (2024, October.).” We would need to remain mindful

management services and dedicated clinics for comprehensive diabetic care
(onsite medical pavilion space/floor dedicated solely to diabetic-related clinics.
Clinics and departments include cardiology, dialysis, endocrinology, laboratory,
nephrology, outpatient pharmacy, and podiatry.
