Introduction
This comprehensive exit assessment examination is designed to
evaluate the advanced clinical knowledge, diagnostic reasoning, and
evidence-based practice skills required for the Family Nurse
,Practitioner (FNP) Capstone Practicum and Intensive course at
Chamberlain University. The exam serves as the final competency
assessment to prepare students for FNP board certification
examinations, covering advanced health assessment, differential
diagnosis, treatment planning, pharmacologic management, and
primary care across the lifespan. Through a combination of multiple-
choice questions and clinical vignettes, candidates are expected to
demonstrate proficiency in managing acute and chronic conditions,
applying clinical guidelines, and making sound clinical decisions in
complex patient scenarios. Emphasis is placed on integrating
evidence-based practice, patient safety, cultural competence, and
interprofessional collaboration in the delivery of primary care.
SECTION ONE: QUESTIONS 1–100
Question 1
A 45-year-old male presents with acute onset of severe, tearing chest
pain radiating to the back. Blood pressure is 100/60 mmHg in the
right arm and 80/50 mmHg in the left arm. Which diagnosis is most
likely?
A. ST-elevation myocardial infarction
B. Acute aortic dissection
C. Pulmonary embolism
D. Pericarditis
B. Acute aortic dissection
, RATIONALE: Acute aortic dissection classically presents with
sudden, severe tearing chest pain radiating to the back, along with
pulse or blood pressure deficits between extremities. This
presentation, combined with a blood pressure differential, is highly
suggestive of aortic dissection and requires immediate imaging
confirmation.
Question 2
A 55-year-old patient with hypertension and hyperlipidemia
presents with a blood pressure of 150/90 mmHg. What is the
appropriate initial treatment for this patient?
A. Lifestyle modification only
B. Hydrochlorothiazide
C. ACE inhibitor
D. Lifestyle modification plus pharmacotherapy
D. Lifestyle modification plus pharmacotherapy
RATIONALE: For patients with blood pressure ≥150/90 mmHg,
the recommendation is to initiate both lifestyle modification and
pharmacotherapy. Target BP for patients >60 years is <150/90
mmHg.
Question 3
What is the first-line treatment for hypertension in a patient with
diabetes and chronic kidney disease?
, A. Thiazide diuretic
B. Calcium channel blocker
C. ACE inhibitor or ARB
D. Beta-blocker
C. ACE inhibitor or ARB
RATIONALE: ACE inhibitors and ARBs are first-line for
hypertension with diabetes and CKD due to their renoprotective
effects. For Black patients, initial therapy may also include CCBs or
thiazides.
Question 4
A patient presents with a blood pressure of 160/100 mmHg. Which
medication class is recommended as first-line therapy for Black
patients with hypertension?
A. ACE inhibitor
B. ARB
C. Calcium channel blocker or thiazide diuretic
D. Beta-blocker
C. Calcium channel blocker or thiazide diuretic
RATIONALE: For Black patients with hypertension, initial
therapy may include CCBs or thiazides. ACE inhibitors and ARBs
are also effective but may be less effective as monotherapy in Black
patients.
