Carboprost (Hemabate)
Carboprost is used for induction of labor, postpartum hemorrhage (PPH),
and midtrimester abortion by directly stimulating the myometrium to
cause uterine contractions. It is a prostaglandin/oxytocic that effectively
facilitates expulsion of the fetus or control of bleeding after delivery.

Mechanism of Action:
It causes uterine contractions by acting directly on the uterine smooth
muscle, leading to cervical dilation and expulsion of products of
conception or blood clots.

Therapeutic Effects:
Control of postpartum hemorrhage

Induction of labor or abortion

Dosing:
Intramuscular (IM): 250 mcg, may be repeated every 15-90 minutes, not
exceeding 2 mg total

Side Effects:
Gastrointestinal: Diarrhea, nausea
Uterine: Uterine rupture (serious risk)

Respiratory: Wheezing (especially in asthmatics)
Fever and chills (shivering)

Contraindications:
Acute pelvic inflammatory disease

, lOMoARcPSD|50131128

Active pulmonary, renal, or hepatic disease
Use cautiously in: uterine scarring, asthma, hypertension, diabetes,
seizure disorders

Nursing Interventions:
Monitor vital signs: blood pressure, heart rate, temperature
Observe vaginal discharge and signs of hemorrhage: increased bleeding,
hypotension, tachycardia, pallor

Assess contraction duration and uterine tone; notify if contractions are
absent or last >1 minute

Monitor for uterine rupture signs and manage accordingly

Additional:
High-risk medication requiring second nurse verification

No specific antidote available

Labetalol
Labetalol is a beta-adrenergic blocker indicated for hypertension
management during pregnancy and postpartum.

Mechanism of Action:
It blocks both beta1 (cardiac) and beta2 (pulmonary, vascular, uterine)
receptors, reducing heart rate and vascular resistance, leading to
decreased blood pressure.

Dosing:
Oral: 100 mg twice daily

IV: 20 mg (0.25 mg/kg), titrated as needed

Side Effects:
Orthostatic hypotension
