glucose self-monitoring records while being treated to get 12 month
certification. The driver then presents with form within 45 days of
form completion for DOT physical. The CME must consider the
information on the ITDM 5870 form. The driver is not qualified if not
maintaining proper insulin regimen or stable blood sugar control. If
there driver does NOT provide 3 months of recorders, the CME can
grant a 3-month certification to allow time to complete the records.
Key note: Determination pending CANNOT be used to allow driver to
get form 5870 completed. Instead, the driver MUST BE
DISQUALIFIED.
