Synopsis

Ms. Henderson, a 21-year-old woman, came to the clinic during this week's Shadow

Health assignment because she had been having painful urination and dark urine for the

past three days. Due in large part to the structure of the female lower urinary system and its close

location to the reproductive organs, women are far more susceptible to UTIs than males

(Czajkowski et al, 2021). The patient also reported having cramps in her lower abdomen,

although she denies having a fever, nausea, vomiting, or back pain. Ms. Henderson’s urine

dipstick for nitrites/leukocyte esterase was positive while her pregnancy test was negative. The

primary criterion used in UTI diagnosis is leukocyte count, so a count of more than 10

leukocytes indicates infection (Czajkowski et al, 2021). Based on the results of my evaluation,

labs, and the patient's stated symptoms, an uncomplicated urinary tract infection was diagnosed.

These types of UTIs may resolve on their own without medical intervention, but many patients—

like the patient in this scenario—will nevertheless seek therapy to relieve their symptoms.

What did I learn?

What I learned during this assignment was how to ask the right questions to determine

the right treatment plan. Treatment for asymptomatic UTIs is not always necessary because

overuse of antibiotics can result in the establishment of strains that are resistant to them

(Czajkowski et al, 2021). So, it’s important to ask the right questions to determine if the patient is

symptomatic or asymptomatic. I particularly appreciated that they had a broad list of different

antibiotics to pick from making it more challenging to decide which one was the most

appropriate choice to treat the UTI. This patient's best option turned out to be nitrofurantoin

because she resided in a place where sulfamethoxazole/trimethoprim combo medications were
