Chief Complaint: "I've been having these really bad headaches over the last few months . I don't have one right
now, and haven't had one in about a week, but I thought I'd have it checked out anyway."
Symptom Progression: "I have been having headaches every 1–2 months for the past 10 years, but over the
past few months they're happening much more frequently—like every 1–2 weeks. The pain is also significantly
worse."
Pain Character & Location: "It's an intense, throbbing pain, usually localized to the left side behind my eye and
temporal region. It does not radiate down my neck."
Visual Aura & Premonitory Symptoms: "Often before the headache comes on , vision in both eyes gets blurry
and spotty at the edges—kind of like zig-zag lines or flashing lights. That lasts for about 20–30 minutes before
the severe head pain starts."
Associated Symptoms: "Severe nausea and vomiting when the pain peaks. I get extremely sensitive to light
(photophobia) and sound (phonophobia)."
Duration & Relieving Factors: "An attack can last up to 15 hours—the whole day is shot. It helps if I lie down in
a dark, quiet room and try to sleep it off."
Triggers & Stressors: "Working part-time as a waitress while being a full-time grad student is super stressful.
They usually happen when I don't get enough sleep or drink 2–3 glasses of red wine a week. Also eating junk
food like chocolate."
Medication Failure: "I used to take acetaminophen or ibuprofen , but now by the time I take it, I can't keep it

SYNTHESIS OF SUBJECTIVE FINDINGS
Classic 10-year history of episodic unilateral throbbing headaches with visual aura (scintillating scotoma),
photophobia, phonophobia, and nausea/vomiting. Recent frequency escalation (from monthly to weekly)
triggered by stress, sleep disruption, red wine, and chocolate.
