Defiance and emotional dysregulation parents married.
Lives in house
with father,

younger siblings,

the way. Any
recent changes in
family dynamic or

at home. In
school, decline in
academics. No
exposure to
smoke. Any
exposure to drugs

Hx Present Illness: (7 Variables but do not list as such) gun locked up?
Patient brought in for defiance and emotional dysregulation that has progressively been getting
worse. The patient has been experiencing this in both school and at home. Per mother the patient
started to experience a change in behavior during the second grade. What grade is he currently in?
Per mother patient becomes irritable and upset when asked to perform an action at home. The
symptoms seem to be worse prior to an event or in the morning. He has been wetting the bed
multiple times a week along with difficulty sleeping prior to the STAAR test. Did the symptoms
resolve after the test, or did they continue? Worsening abdominal pain in the morning, but not at
night. Does he have abdominal pain on the weekends? Reports pressure from parents to perform
well in school but doesn’t think he can meet their expectations. Mother what are your guy’s
expectation of James in school and at home? Decline in academics and misbehaving at school. He
does report bullying that started last year. What kind of bullying is happening at school?
