, ICU PATIENT MANAGEMENT
HEMODYNAMIC MONITORING
Hemodynamic monitoring is a critical
aspect of ICU patient management,
providing data about cardiovascular
function and guiding treatment decisions.
Below is a detailed description of each
type of hemodynamic monitoring,
including its purpose, components, and
nursing implications.

KEY COMPONENTS
Key Components
-Central Venous Pressure (CVP): Measures preload (right atrial pressure) to
assess fluid status.
Normal range: 2-8 mmHg.
High CVP: Indicates fluid overload or right heart failure.
Low CVP: Suggests hypovolemia or dehydration.
-Arterial Line Monitoring: Continuously measures blood pressure and allows
for arterial blood gas (ABG) sampling.
Nursing Care: Maintain patency, ensure zero referencing, monitor for
complications (infection, bleeding).
-Pulmonary Artery Catheter (Swan-Ganz):
Measures cardiac output, pulmonary artery pressures, and systemic
vascular resistance (SVR).
Nursing Care: Monitor insertion site, ensure waveforms are calibrated, and
assess for arrhythmias.
-Cardiac Output and Stroke Volume:
Cardiac output = Heart rate × Stroke volume.
Monitoring devices include Vigileo and PiCCO.

,HEMODYNAMIC MONITORING
CENTRAL VENOUS PRESSURE (CVP) MONITORING

(CVP) MONITORING
Purpose
CVP measures the pressure in the
thoracic vena cava or right atrium,
reflecting right ventricular preload
and overall fluid volume status.

NORMAL VALUES: 2-8 MMHG

Clinical Significance
Elevated CVP: Indicates fluid overload, right heart failure, or
pulmonary hypertension.
Low CVP: Suggests hypovolemia, dehydration, or blood loss.
Components
Central venous catheter (placed in subclavian, internal jugular, or
femoral vein).
Pressure transducer and monitoring system.

Nursing Implications
Ensure proper zeroing and calibration of the transducer at the
phlebostatic axis (4th intercostal space, midaxillary line).
Monitor for complications: infection, thrombosis, pneumothorax.
Assess waveform for abnormalities (e.g., dampened signal may
indicate catheter occlusion).

, HEMODYNAMIC MONITORING
ARTERIAL LINE (A-LINE) MONITORING

PURPOSE
Continuous monitoring of arterial
blood pressure (ABP) and access for
frequent arterial blood gas (ABG)
sampling.
NORMAL VALUES
SYSTOLIC BP: 90-120 MMHG
DIASTOLIC BP: 60-80 MMHG
MEAN ARTERIAL PRESSURE (MAP): 70-100 MMHG
(GOAL IN CRITICAL CARE: MAP > 65 MMHG)

Clinical Significance
Provides real-time BP measurements in patients requiring
titration of vasopressors or inotropes.
Accurate evaluation of blood pressure trends.
Components
Arterial catheter (commonly inserted in the radial, femoral, or
brachial artery).
Pressure bag and transducer system.

Nursing Implications
Perform Allen’s test before radial artery insertion to confirm
collateral circulation.
Ensure pressure bag is inflated to 300 mmHg to prevent blood
backflow.
Monitor for complications: bleeding, infection, and distal ischemia
(check capillary refill and pulses distal to the insertion site).
Prevent waveform damping by ensuring proper positioning and
patency of the catheter.
