History of Present Illness (HPI)
Layla Stevens is an 18-month-old female brought to the clinic by her mother due to a 24-to-
36-hour history of vomiting and frequent watery diarrhea.
Onset & Duration: Symptoms began approximately 30 hours ago after returning
home from daycare.
Vomiting: Mother reports 5 episodes of non-bilious, clear-to-yellowish fluid
yesterday. Today, Layla has vomited twice, typically within 15 to 20 minutes after
attempting to drink fluids or eat. No hematemesis or coffee-ground material
observed.
Diarrhea: 6 to 8 episodes over the past 24 hours consisting of voluminous, watery,
yellow-green stools. No gross blood, mucus, or dark tarry stools noted.
Fever: Low-grade tactile fever noted at home yesterday evening
($100.4^\circ\text{F}$ / $38.0^\circ\text{C}$). Mother administered one dose of infant
acetaminophen, which reduced the temperature. No current fever reported today.
