Introduction
A successful doctoral project requires more than just identifying a gap; it requires
structuring it into a clearly defined, realizable implementation plan, considering factors such
as institutional dynamics and resources, and the clinical results sought. The implementation
plan will support the doctoral project "Improving Medication Reconciliation and Adherence
to Reduce Readmissions in a Psychosocial Rehabilitation Setting." The project centers on
adults with severe psychiatric disorders in transition to outpatient care. The project aims to
enhance medication adherence and decrease preventable psychiatric readmissions through
nursing interventions comprising medication reconciliation via the Medication
Administration Record (MAR) and patient education using the Teach-Back Method from the
AHRQ Health Literacy Toolkit. The plan greatly enhances the previously developed project
charter and the QI/PI framework using the Plan-Do-Study-Act (PDSA) model, considered
one of the most celebrated frameworks for iterative clinical improvement (Moser et al.,
2020).
Project Intervention and Implementation Description
The quality improvement intervention consists of a dual-pronged strategy integrating
structured medication adherence support and staff-facilitated patient engagement to address
contributors to thirty-day psychiatric readmissions. Medication adherence monitoring will be
embedded into the routine operations of the psychosocial rehabilitation (PSR) program.
Licensed nursing staff will engage patients during biweekly sessions to review current
medication regimens, discuss usage patterns, and identify potential adherence barriers. Staff
will guide patients through clarification of prescription details and address misunderstandings
using verbal reinforcement and structured documentation tools.
