1. Failure is or isn’t an option, why?
In Human Centered Design, failure is actively sought out as a preventative
measure for the future. If those failures are never experienced, lessons in
prevention or mitigation will never be learned.
2. What is the purpose of Human Centered Design (HCD)?
HCD aims to mitigate failure points that could result from human interaction,
account for all human factors prior to the completion of a product, and create
designs centered not only on the operator but also the environment they are
operating in (both physical and cultural).
3. How might Heinrich’s Law explain disasters as they pertain to Latent Failure?
Disasters are comprised of a chain of events that can be stopped by removing a
single link, similar to a line of collapsing dominoes.
4. Both Complacency and Norms (Normalization of Deviance) can be the most difficult bias to
acknowledge. Why might this be true?
These biases develop over long periods of time, perhaps only getting worse in
unnoticeable increments (just like the cleanliness of my glasses most of the
time).
5. What are the three ‘Fs’ of a startle (fear) response?
Fight, Flight, or Freeze
, 6. Considering Invisibility Training, how might Saccades, Selective Attention, and Peripheral,
Beam, and Contrast Blindness(es) contribute to a disaster?
Saccades and Selective Attention contribute to disasters due to the brain fixating
its attention on certain things at a time. Saccades are able to block things from
our brain during quick eye scans while selective attention gives us tunnel vision.
Peripheral, Beam, and Contrast blindness contribute to disasters by making
drivers “blind” in a way. It’s very difficult to focus on something either in our
peripheral vision, something that’s being blocked from our vision by something
else, or something that blends in with its surroundings.
a. Are Heuristics involved?
Absolutely. For example, we are not going to focus nearly as much when driving
in our own neighborhood as opposed to a city we’ve never been to before.
7. How do Motion and Projectiles Differ? Consider the flow of energy.
With motion (and more so the sudden lack of motion), a human body could
become a projectile itself with a lot of kinetic energy that could impact
something causing an injury (for example, falling off a ladder). Projectiles could
come from the environment in the form of debris, shrapnel, or dislodged/failed
structures. In this case, those mechanisms of injury possess the kinetic energy
and could possibly hit anybody in vicinity (for example, getting hit by a piece of
flying metal from an explosion).
