Wellness and Disease Prevention:
Reflect on the health promotion disease prevention interventions you
witnessed in your practicum site, as it relates to the social determinants of
health most prevalent in your community. What did you see? What does
this time mean to you as a professional nurse in your role?
Your response:
During my practicum at a medical detoxification facility, I observed how
healthcare technology plays a vital role in supporting health promotion and
disease prevention, particularly in relation to the social determinants of
health affecting this patient population. Many patients in this setting face
challenges such as limited access to healthcare, substance use disorders,
unstable housing, and lack of consistent support systems. The use of the
Electronic Health Record (EHR) allowed nurses and
interdisciplinary team members to track patient histories, identify patterns
of substance use, and monitor withdrawal symptoms using tools like CIWA
and COWS. This real-time documentation supported early intervention and
prevention of severe complications, such as seizures or worsening
withdrawal symptoms. Additionally, tools like the Prescription Drug

Monitoring Program (PDMP) were essential in identifying patterns of
controlled substance use, helping providers make safer prescribing
decisions and prevent further misuse. Patient education was also
supported by technology, as nurses documented teaching and reinforced

also noticed that usability challenges and time-consuming documentation
sometimes limited the time nurses could spend directly engaging in
preventative education with patients. This experience highlighted for me
the importance of integrating technology into preventative care while still

In the detoxification setting, I observed chronic disease management
through the lens of substance use disorders, which require long-term,
coordinated care. Healthcare technology played a key role in supporting
interprofessional collaboration among nurses, providers, and behavioral
health staff. The EHR allowed for seamless communication, where each
discipline could document and review patient progress, medication
adjustments, and treatment plans. The use of eMAR ensured accurate
medication administration, which is critical in managing withdrawal
symptoms and preventing complications. I saw how nurses relied on vital
sign monitoring equipment and integrated assessment tools to
continuously evaluate patients and communicate any changes to the care
