“Loose stools for the past three days.”
History of Present Illness (HPI)
The patient is a 25-year-old adult who presents with a three-day history of acute
onset loose stools. The patient reports that the symptoms began approximately
three days ago and have progressively worsened. The patient is experiencing six
to eight episodes of watery diarrhea per day, described as mostly watery in
consistency. The patient denies the presence of visible blood in the stool but
reports possible mucus, which raises concern for an inflammatory or infectious
etiology.
The patient endorses a strong sense of urgency associated with bowel
movements and states difficulty holding stool once the urge begins. The patient
reports lower abdominal cramping pain, intermittent in nature, which improves
temporarily after bowel movements.
The patient recently returned from a trip to Mexico, where they consumed street
food, representing a significant risk factor for traveler’s diarrhea. Associated
symptoms include low-grade fever, chills, fatigue, and one episode of vomiting
two days prior to presentation. The patient denies ongoing vomiting but reports
poor oral intake and inadequate hydration, stating they have attempted to drink
water but do not feel it has been sufficient.
The patient denies recent weight loss, recent antibiotic use, and has no known
history of chronic gastrointestinal disease. There is no reported history of similar
episodes in the past.
