In the Med Room
✅ Get report and identify:
● Patient name
● DOB
● Doctor is ___
● Allergies (NKDA)
● MR number
✅ Check that everything matches both orders + MAR
● Now look at the time ex: 0800
● Look at what we’re giving at that time
✅ Perform safe dose range calculation
● Round as instructed (nearest tenth)
● Verify dose is within safe pediatric range if required
✅Pull out meds + gather supplies (med cup, alcohol pad, oral syringe)
First Medication Check - With MAR
When pulling medication:
"I am comparing with the provider's MAR.”
Verify:
● Right patient
● Right drug
● Right dose
● Right route
● Right time, document later
● Expiration date | Medication not tampered with
Do Dosage Calculation: “Calculate how much to administer using label (mL, tab, cap, etc)”
, Correctly Draw Up Medication
For vials:
"I am cleansing the vial with an alcohol pad for 15 seconds and allowing it to dry."
Then:
● Draw up medication
● Remove air bubbles
● Recap using one-handed scoop if required by instructor
● Label immediately as syringe leaves your hand (label as I go)
Oral Medication “Mix up Medication”
Say:
"I am measuring the medication at eye level to ensure accurate dosing."
Labeling
Include:
● Medication name
● Amount drawn up mL
● Initials
● Date
● Time
● Expiration date
Second Medication Check
Before leaving the med room and after drawing up meds + putting on labels:
"I am comparing the medication label to the MAR for my second medication check."
Then verify:
● Right Patient
● Right Drug
● Right Dose
● Right Route
● Right Time | document now
● Med isn’t expired or tampered with
