Scenario: DM 2 with Gestational Hypertension
Date: 9/22/24
1. How did that feel? e.g. stressful, good, frustrating, energizing, exciting, etc.
The scenario was definitely challenging. The patient, a 27 years old woman, came in with high
blood pressure and history of non-insulin dependent type 2 DM. Her case felt stressful and
required immediate attention because of her increased risk of developing gestational
hypertension and rule-out preeclampsia. During the assessment, she reported having a headache,
which we treated with Tylenol, and thankfully that helped. However, her blood pressure
remained consistently high above 150/90, in two separate readings, which made the situation feel
ever more urgent. I couldn't help but feel the pressure, knowing that in cases like this with quick
and effective communication can have a huge impact on the patient’s outcome.
2. Talk about three things that went well. Why did they go well?
First the patient education provided was through to her specific needs. Patricia’s elevated blood
glucose and blood pressure, it was important to educate her about managing both conditions to
prevent complications during her pregnancy. Second, the use of electronic fetal monitoring was
effective. Monitoring the fetal heart rate is always important part of assessing the baby’s well-
being which helped to alleviated some concerns and allow us to focus on patient’s care. Lastly,
the reassessment of her headache after giving Tylenol was a success. I check back with Patricia
to see if the medication helped, and it was relief to hear that her headache had improved.
