Be sure to address the following in the first supervision meeting and then as
needed:
1. I have reviewed with my Field Instructor the specific tasks that I will be doing
during my placement. ☒
2. I have reviewed with my Field Instructor specific professional expectations. ☒
3. I have reviewed my schedule with my Field Instructor. ☒
4. I have reviewed “exceptions” to the schedule—including inclement weather,
illness, and absences—with my Field Instructor. ☒
5. I understand that if there are changes to my placement or Field Instructor, or
if there are performance issues, I will contact my Course Instructor
immediately. ☒
