N.C.a 52-year-old Female considered a reliable historian
Father (alive): Coronary artery
disease
Mother (deceased): Cancer
Sister (alive): diabetes
Son (Alive): heart block
Personal/Social Hx:
Chief Concern/RFE: N.C is married and lives with
“bad reaction to mosquito bite” husband. Son is out in college and
visits often. She also has pet dogs
and lives in a house. She does not
smoke and occasionally drinks
alcohol. She is and her husband
are both employed and denies any
needs related to housing or food.
Likes to travel and spend time
outside in nature.
Hx Present Illness: (write in paragraph format)
N.C is a 52 year old female with history of hidradenitis suppurativa who presents with painful 7/10 enlarging lesion
on her back after presumed mosquito bite 4 days ago due to increased exposure in her yard. Most lesions have
resolved but one lesion on her back has progressively enlarged to about golf ball size, with increasing pain and
discomfort. She cannot reach or see it so is unsure if the bump has been present prior to the incident. She has tried
warm compress without relief and applied OTC topical ointment for the first time today. She denies any fever or
other systemic reactions.
